Because you have prior information of who the actor is so you end up making an association. Without any context or any knowledge of who is behind the makeup, would you really think of Heath Ledger or some random old dude when you see this? The makeup and scar prosthetics do a good a job of...

By my recollection, you don't really see much of Heath's likeness under the Joker makeup except for the scenes of him asking how he got those scars. I think it's best to compare the sculpt based on character likeness rather than perceived likeness of Heath under all the makeup and prosthetics.

I have my mk 85 displayed in front of me. The lower torso is from the 85.

That's the initial/inaccurate mk 85 HT. The features were updated since Disney can't stop doing post production updates on the cgi. Anyway, the lower half of the torso is from the 85. So the head with upper torso, lower arms, and lower legs are new.
