The first one is a woman (assuming). The second is a man.

If you're born a man/woman that's what you are. If you change your sex, cool. No problem with that. But that doesn't mean you magically erase what you were born as. Not to mention that "girl" still has a ____.

Gender and sex are two different things. Gender, as it is understood by science, is a mental thing. Sex is a physical thing, dealing with anatomy. It's possible for these things to not match up, and that's what causes people like the two in my image to change from one to the other.
Gender is what makes you a man or woman. sex is what makes you male or female. the terminology is not perfect, and these terms are not mutually exclusive to each other.
