Nope, I think it was replaced by the keychain (raw cast) that he included.

Truetype Narrow Shoulder is probably the best solution. Pretty sure that's what Rainman himself used.

I think the problem there isn't the headsculpt, but a proper body and then clothing that fits a body that size. I don't see it being possible atm.
