Quite easy, and yes. All you need is some hot water and tweezers.
Heat up some hot water, remove the head sculpt, hold it upside down and pour the water inside the head hollow. After 30 seconds to 1 minute, dump out the water and use tweezers to pull out the eye stems from the inside. The hot water allows the face to relax and the eyes to pop out with minimal effort. Also, dry out the interior of the sculpt as you won't need to use water again.
Once the eyes are removed, I recommend cutting the stems at the back of the eyes down a little bit. If the stems are too long (like they are by default) then the eyes will move out of place whenever they're bumped by the neck connector inside the sculpt, which is quite annoying. You want enough slack for them to still be grippable with tweezers, but not so much they get bumped by the connector whenever attaching the head to the body.
Lastly, use the tweezers to guide the eyes back into the sockets from the inside and Voila! This was the exact process I used to swap the eyes from the StarMan Sculpt to the PlayToy Sculpt.
