QC, they accidentally came out and stayed in the head. It looked funny so someone took a pic. Hot Toys doesn't have a great history with glue.

The teething mouthpiece is growing on me.

The extreme open mouth is great (as in the teeth look like real teeth) not sure I would ever pose with it though.

I'm baffled why HT has not done a 1/4 Iron Man yet........weird!

Maybe because HT still not done milking the 1/6 iron man line yet?
They're pretty much done making the 1/6 batman line, but not with iron man. If they announce any 1/4 iron man stuff, at least I will put hold to continue purchacing them mindlessly. Not to mention Pi right now has the license for 1/4 iron man
