The eye placement there is making him look really weird. He's looking down on Thor so it looks weird. Have them looking up and I imagine it looks a lot better. He looked fine in the other trailer anyways.

Honestly? I love it. While Ultron Prime gives me similar vibes to his humanoid forms (with a touch of his modern stuff), this one definitely looks more closer to his modern designs, what with the smoothed-out head/helmet.

While some might think it looks derpy (can't deny that Ultron's classic design sometimes does as well), I think it looks more unsettling.

No one knows. No one has in fact seen the movie. And they came up with those names so no one could know. They could have used much less ambiguous names.

It seems they are the same thing, just that at some point Prime seems to think that having a body made of metal is not enough so he decided to get more armor to cover his chest and face

Yup. Whether it's an entirely new body is still up in the air, but it basically looks like an armored Ultron Prime.
