Also, as for Jack's top billing, it was and is still common for the biggest name to get top billing. Take a look at Superman, both Marlon Brando and Gene Hackman's names appear before Christopher Reeve.
That might also be a SAG requirement too, not necessarily just the actor's desires.
It's not based on SAG requirements. The way top billing works is that the actors and actresses are listed in decreasing order of salary. Usually, that means the main star comes first, but often (as in the case of Superman 78, and Batman 89), the "supporting" actors are bigger stars than the lead, command more money, and thus, top billing.
An exception to the listing the actors by decreasing order is the "...and" credit. The salary of the actor listed there is not the lowest paid actor on the film, despite their name appearing last. For example, there is no way Katie Holmes got more than Morgan Freeman for Batman Begins.
Ok, what about Terence Howard in Iron Man then? He was the highest paid, but I don't think he got top billing.
Howard was the first actor signed to the film and, on top of that, was the highest-paid. That's right: more than Gwyneth Paltrow. More than Jeff Bridges. More than Robert Downey Jr. And once the project fully came together, it was too late to renegotiate his deal. It didn't help that, according to one source, Favreau and his producers were ultimately unhappy with Howard's performance, and spent a lot of time cutting and reshooting his scenes.
As I understand it, the producers kinda bungled the signing and salary negotions for Terence Howard.

It's funny that it says people were unhappy with Howard. I think he was miles better than Cheadle in that role and actually had chemistry with RDJ.
100% agreed. Cheadle is easily the better actor, but his Rhodey sucked and his chemistry with RDJ was nearly non-existent compared to Howard's. The scene on the Stark jet in the first film alone blows away anything Cheadle did in IRON MAN 2. Such a shame, that recasting.
