I thought I read in the Making of Star Wars that it was from (indeed what Wor-Gar said) Death+ Dark= Darth Invader=Vader
So even George doesn't know what his original idea was

The Dark Father thing Lucas has been claiming since the Special Editions at least. I remember reading and seeing interviews around the time of ANH re-release into theaters that it was Dark Father, I doubt you'll get him to admit to anything else now.
