I'd have to go with the Chainsaw guys from Resident Evil 4 and 5. I remember walking into a new area in 4 and if I heard the distant hum of the chainsaw I'd be piss scared.

Poor Brad... Nemesis was one fo best he appeared out of no where and was instant death if your werent always on your toes... I remember running like hell in the other direction a good deal of times...
