No pre conditions need to exist.

but inside the cranium swelling begins.

Do any of us see the immediate results of being bruised on our arms or legs?
No.
Usually it takes hours.
This is correct. I knew a guy who hit his head hopping a fence and died the next day. He was perfectly fine all night except for a bump. Next morning he wakes up and just passed out. These brain injuries happen in war all the time. An explosion happens, everybody seems ok, next thing you know everyone is getting sent to Mash.
