Does anyone give him a challenge at all or does he just cut thru everybody with zero effort.

Once he goes into Rambo mode, no one stands a chance, but he's not fighting people with martial arts. It's basically the forest sequence from First Blood or the jungle sequence in part 2, both in the second act and final act. The difference is that he's actually angry and he wants to torture the bad guys. It shows his skill while keeping it age appropriate, even though Sly at age 73 still looks like a freak on screen. He not only looks huge, he looks like he could easily snap anyone's neck with his hands,.even if they are much younger.

Rambo 3 is more iconic, but Last Blood has better villains and acting than 3 and 4. Also, you actually care and want to see Rambo kill the bad guys unlike part 3.
 
I can tell you there are two versions of this film. The one released in the USA is shorter.

The other version has a 10 minute opening in a rain storm where Rambo tries to rescue a couple in a muslide of sorts and only saves one.

Another short scene where they show the corrupted mexican police that also participate in the rape of slave women.

So about an extra 10-15 mins was cut from the USA release
 
Who cares. If you even remotely like the Rambo films I'm sure this will be more than enjoyable.

It was, watching rambo setting up his traps with the Rambo 2 theme music trumping was great!

I remember the set photos showing the storm and forrest scene. My guess is that they didn't add much to the main story. I'd like to see it. This film could be better if directed and edited by an A list level filmaker, because the acting is good and while the story is nothing new, it's effective .by making you care about the main characters. Clearly they need it a bigger budget. Those green screen shots were embarrassing. Nothing like that in any of the previous movies.

Had to of been purposely done for an extended cut dvd release or something. I saw both versions. In a way,that opening rain/forest scene didn't feel like a Rambo movie at all. Reminded me of just another movie with stallone in it,but I can see they were trying to make John Rambo as if he wasn't just doing nothing while just living on a farm training his horse all day long. The police grabbing the girls to rape them,I am glad that was cut out as we got the point already with the scum brothers selling them as sex slaves.
 
That's the worst, and what I feared. I know one of the producers and when I saw he was involved I feared immediately it would look cheap.

The cheapness of it is the most disappointing part for me. I kid you not, the second scene of the film is a moving shot inside Rambo's house where we see photos and old weapons, and it was blurry. Also, too many close ups when people are talking. Two scenes had Rambo and the girl driving and you could tell the background was a green screen.The violence and blood looked fine tho. Not every scene is.bad or cheap, but occasionally you see one through out the movie.
 
Clearly this film ran out of money at some point during production and some unusual producers were brought in for finishing funds. I imagine there's also a very uneven look to it -- as if its two movies meshed together: a movie movie and a tv movie.
 
I'll use spoiler.

I thought it was pretty rad when Rambo ripped that guy's heart out with his hand. I don't know if it's physically possible, but a lady behind me shouted, "holy ****!" It was hilarious. Unfortunately, that was a missed opportunity for some real drama and good acting.

I feel that he should have kept the guys heart when he goes and sits on his rocking chair. It could have been a very dramatic scene, where Rambo realizes that it was all meaningless, because nothing is going to fix the damage that was done by those bad guys. At that point, as he's still holding that heart, he could have had a moment of reflection or maybe a mental breakdown like in First Blood. It could have been a very artsy moment, but instead, the cutting of the heart was played like something out of Commando, like a "Yeah, that's badass bro!!" moment.
 
Who are the key bad guys, and how does Rambo kill them?

Two brothers that control a sex trafficking ring. They're not a cartel, like the reviews call it. Rambo goes into one of the brother's house, kills everyone with his knife silently, and then cuts the brother's head off. As he's heading back to Arizona he throws the guy's head and it lands on the road. He leaves a photo in the guy's house of his nice so that the other brother knows who did it. They had his ID, so they knew where he lived. Rambo uses his bow and pins the other brother to a wall with arrows, then walks up to him and stabs him on the chest with his new knife, slowly cuts him open, then sticks his hand into his chest and rips his heart out. There were lots of heart references through out the movie.
