Those commercials are annoying, not just because it alerts you to a twist, but because I feel like it cheapens the story by making it seem as if the twist is a gimmick to get people to watch as opposed to a part of the story that would occur no matter what. Of course maybe the twist is a gimmick to get people to watch if ratings have been dropping. Of course that could also be why they advertised it that way, to get people who haven't been watching to tune in.
That worked for me in one of the best ER episodes I've seen (which isn't many). The twist was also done in the best way I've seen too. It was the Valentine's Day episode and everything before was enough to make me forget there was a twist coming (which is why I tuned in) and then that final twist came and it was pretty shocking. Great episode imo.
